Description: Plaque on plain wood pattress. There is border pattern of vine leaves and grapes. The dedication is in Roman capitals, the names are in sans serif capitals, and the words "Faithful unto death" are in Gothic upper and lower case
Inscription: IN GRATEFUL MEMORY/ OF THE/ SONS OF THIS CONGREGATION/ WHO GAVE THIER LIVES/ IN THE GREAT WAR/ 1914 - 1919/ (NAMES)/ "FAITHFUL UNTO DEATH"
