- A plain rectangular brass plaque with an incised dedication in black lettering. The badge of the Royal Field Artillery is incised at the top centre of the plaque.

- THIS TABLET IS ERECTED IN PROUD MEMORY OF/ CAPTAIN WILLIAM HENRY WILSON SMITH, M.C., R.F.A./ OF COLEBROOK PARK./ HE WAS SEVERELY WOUNDED IN THE GREAT WAR, BUT/ BY HIS COURAGE AND DETERMINATION HE OVERCAME/ ALL PHYSICAL DISABILITIES, AND LED AN ACTIVE AND USEFUL LIFE. HE DIED FEBRUARY 16TH 1928, AGED 38/ YEARS, LEAVING A TRUE EXAMPLE OF GOODNESS, PATIENCE AND HUMILITY./ HE WAS A FREQUENT WORSHIPPER IN THIS CHURCH./ "BLESSED ARE THE PURE IN HEART FOR THEY SHALL SEE GOD."/ "THE PATH OF THE RIGHTEOUS IS AS THE SHINING LIGHT THAT/ SHINETH MORE AND MORE INTO THE PERFECT DAY."
